Kindergarten Registration
April 1 - April 16
Dear Parents,
La Porte Independent School District will hold registration of kindergarten students for the 2004-2005 school year during the weeks of April 1 - April 16th. All children who are eligible for kindergarten next year should be registered by a parent in the office of the elementary school for which they are zoned.
THE FOLLOWING INFORMATION WILL BE
REQUIRED AT THE TIME OF
KINDERGARTEN REGISTRATION
- Birth Certificate or certified photocopy verifying that the child will be five years old on or before September 1, 2004.
- Shot Record verifying up-to-date immunizations. The following immunizations are required before beginning school:
| DPT/DTaP | 4 doses, including one received on or after the 4th birthday | ||
| POLIO | 3 doses, including one received on or after the 4th birthday | ||
| MMR | 1 dose, received on or after first birthday and one additional dose of measles vaccine which may be received as a part of second MMR | ||
| HEPATITIS B | 3 doses | ||
| VARICELLA (Chicken Pox) |
1 dose on or after first birthday |
- Social Security Card
- Proof of Residency (Utilities statement, etc.)
